help: Lousy discharge planning, lousy intake


Had a very difficult admit this w/e and I was really PO'd at my supervisor who took the referral (and didnt get me half the story or orders I needed) AND at the d/c planner who did a terrible job sending the info I needed to care for this lady.... on a weekend, a Holiday weekend.... Not only did she have a chest tube with a pleurevac draining and no orders on that at all, she had a very large ulcer on her leg- this I didnt even know about till I got there and the daughter handed me detailed wound care orders (for stuff we dont even have in stock at our hospital, let alone in my bag!) she came home with no supplies...........The poor woman is a brand new CA diagnosis, 30 miles out in the country and Im looking like I dont have a clue. I "made it all up as I went along" and she got good care, but Geez, I dont even know who to be madder at!

I did make some rather pointed calls and documented the heck out of the whole thing but it sucked up my whole day. How can I get my supervisor to send a "what the heck??" form or whatever to this d/c planner? Its our regional hospital, 2 hours away and they have been getting worse.....
