if you have that many years in Med/surg experience you should ACE the test. Make sure you remember the important findings:PT/INR limits, K+ levels, etc.
When you get the job, because I'm sure you will, make sure that you get hired with a company that is willing to TRAIN you. There are so many "rules" about home health. I learned a lot from trial and error. Take your time, don't rush to get out on your own. A good company will be happy with doing that. In the long run it will save them time and $$
