60 visits a week quota ?? sound possible?

Ive been offered a job with a home care agency to do sixty visits a week per their quota. It will be for a RN case manager with mix of mostly med admins, SOCs, recerts, transfers and discharges. ... The company seems to think I wont have a problem at all meeting that quota and most nurses go well over the quota and earn extra money for going over. I will have to do 12 visits a day ... some of behavioral health med admins and and some a skilled visits ..

but most agencies here in CT are requiring 25 or 30 visits a week ..making five or six visits a day... which sounds alot more possible than sixty visits..

do you think i will be able to handle sixty visits a week, i will be sallaried, and the salary includes my money for gas.

Twelve a day sounds like a lot even if they were in the same building. Forty min per pt which includes travel time and paperwork. Maybe you can talk to another employee about how they do it.

No way! not unless they are next door to each other. You would have no time to chart or even stop and take a Pee break! We see 6-7 pt a day. Est. 1 hr with each pt and it should include traval time. Ask the other nurses how the make 12 pts a day. Good luck!

Thanks for your replies ... The agency seems to think that I should be fine because they are mostly med admins ... But it does include recerts, weekly vitals and wound measuring ect :..I will definitely ask the nurse who I will be shadowing how she does it.. But I also think 60 visits is too much in a 40 hour week ..do u guys have med admin visits or prepourin ur mix of daily visits? What kind of patients are u seeing daily?

No. That is double what we excpect. DOUBLE. And that salaried thing is handy, isn't it? You'll be working tons of OT.

Salaried jobs especially for things like this is just an excuse not to give you OT. But 60 visits a week? That's just horrifying.

I don't have any experience with home health nursing but this doesn't seem to pass the reasonableness test.

Assuming this is a typical 40 hour week, each visit - including travel time - would have to average less than 40 minutes. Let's say that your visits are relatively close to one another, no more than 5 miles. Travel time for each visit at an average speed of 25 mph would be about 12 minutes. Let's add 3 minutes to that for the walk from the car to the house, getting the paperwork and supplies ready, etc., making 15 minutes. That leaves 25 minutes for each patient. Is that enough time to get a set of vitals, do a focused physical exam, administer meds or wound care and chart everything? My experience is in pre-hospital, emergency care and 25 mins is not all that much more time than my ambulance crew of 3 spends with a medical transport patient.

Frankly, this sounds like a recipe for you to work for 60 - 80 hours per week. Since it's a salaried position, that's a great deal - - - for your employer.

60 visits a week you say is double that expected by other agencies, salary including gas.

Why did you accept this job? Its a great deal for the agency and a huge rip off for you.

did vna for a few years and our daily quota was 7 pt.s..any more than that-and it was your choice-u got perdiem pay. 60 visits-sounds like an open door for confusion and errors. all u need is to arrive at one home and fine u have to make a few calls, pt sicker than the last visit or whatever and u will be on the road for 16 hours that day. what are your state minimums and max. for public health home care visits. good luck

Thanks everyone for replies and great input!! i shadowed with a nurse today, and visited her patients with her.. she had about 9 med admins and 3 skilled visits.... the med admins were fairly quick...they were title 19 patients here in CT who are behavioral health patients that are non compliant with their meds so they need the meds adminstered by a nurse... being a case manager she has to order meds if they run low, re stock supplies, do recerts , admissions, transfers, discharges , she starts at 6 am and finishes her visits by 1pm, and then goes home to tons of charting...

i think im going to tell this place that i just want to do to per diem... they r in for their profit...the lpns go well over twelve visits some do as much as 18-30 med admins a day which include bid meds...but they dont have the case management portion, and they work crazy hours... i cant see myself doing that and risking my license, and being a RN i would have to do the case management part which would be really hard..

I do hospice home health but only four or five visits per day. Twelve sounds ridiculous however we don't do any quick med admin visits.
