NNP (Neonatal Nurse Practitioner) Jobs

I have read the forum and have not found answers to these questions any help would be appreciated. So here we go. Looking for an NNP job in Hawaii but all I can find is a per diem job in Honolulu. It appears there are no jobs on the other islands. So here are my questions

1. What do they do with sick babies before they are flown out to Oahu?
2. Would out islands hire an NNP to help staff the newborn nursery?
3. How often do full time NNP jobs open up in Oahu?
4. How hard would it be to commute back and forth from the Big Island to Oahu?

thanks
