USPHS - Commissioned Corps

Hello,

I am a nurse practitioner looking at getting into the PHS - (when it opens).

From experienced applicants - I am reading about the medical review - Do they ask for your insurance information, or a release to see your health records? I am looking this part up to see exactly what will need a waiver/qualify for a waiver/what is disqualified. I thought I read somewhere on here that they request this information.

I just took a federal job in hopes that I can convert when PHS opens. However, I dont want to go through this application process and find out I wont get in.

Any help would be appreciated.
