What happened to signing your own initials?


It is to my understanding that when a Nurse gives meds of does a treatment. That this person is to sign there initials in the Medication or Treatment Assessment Record. Where I work the Director of my dept. will sign her initials so that the box is not left blank when someone forgets to sign. Instead of dealing with that person and have them sign when they come back. She thinks by signing her intials is better. Even if she has not worked that day or even the shift. Please tell me what is wrong with this picture. I have only been a Nurse for 17 years. And according to her when I tell her this is wrong. She says I don't know what I am talking about. Or maybe she is waiting for the State to catch her. Who knows.

I would not sign a med sheet like that??? What is her logic?

I worked a place where the DON did that. She said it was to fill in the holes so state would not cite them. She said they would not question her giving the meds and that it was better than holes in the MAR. She did not like conflict. She is still there. I am not.

I ask the nurse who verified by phone that the mad was given to sign her initials/coworker's initials and then chart in the nursing notes that she verified her coworker gave the med by phone.
