Pick up the phone and call the board of nursing and report the LPN signing her name as a RN.
Call whoever certifies NH /LTC (hospitals have Joint Commission, do they do LTC, too?), and report that the LPN is signing her name as an RN. I'm an LPN, and I am FURIOUS that someone who can't pass her boards is signing RN. That crap's got to stop. They can dance around why you left all they want. If the governing body sees one record where an LPN signed as an RN (much less a repeating pattern of it), they can't explain that one away. You don't have to ID yourself, you can just be a "concerned coworker" and couch it in terms that you think the LPN doesn't understand the gravity of what she's doing. Nothing about you, nothing about their relationship, just focus on the one thing they can't explain away -- ink on paper.
