State Report??

I was wondering how one would go about receiving a copy of a nursing home's most recent State review report?

In Connecticut you can view it online by going to the Dept of Public Health.

I know the NH have to post it publicly and I would think you could get it off the state's website.

Try this one. It has the rating and deficiencies at the bottom after you find you nursing home by zip code, name, etc.
