Skin Risk Assessments


Do any of you use Braden, and if so how do you take into consideration multiple co-morbidities that may put a resident at a high risk for skin breakdown, other than what is on that Braden scale?

We use the Norton Plus and base our skin protocol on the results. Personally I think the Braden gives a better indicator of risk but in Massachusetts the Norton rules.

We use the Braden scale. We do a risk adjustment to high risk and place the resident on our prevention program (ie weekly skin inspections, heel prot, turning, etc). However, their actual Braden Scale number remains the same. We use the blank box at the bottom of the last column to write in "risk adjusted to high" d/t whatever co-morbs they have.
