Does your faclity have a policy in place? I'm not sure if we do. What I do is of course check all the basic info..name, code status, docs, allergies. Then go thru the orders until you get to the last months recap. Make any changes needed and make sure I check the lab section. Were labs done or ordered if needed? (Dig levels, TSH, etc)

As far as making corrrections....using a highlighter..block off anything D/Cd and put next to it the date it was done and inital it.

We also initial anything we checked.

I have always taken the New Mar , old MAR New MD orders and the chart and compare all at one time. It takes me about 5 min a piece , this way I can catch the new orders that have not been put in by MR because they were wrote at the end of the month and I can check other not-so new orders to see if they have been put in correctly. I also recommend having one of the regular floor nurses to the checking because these nurses work with these patients every day and very often take and sign off orders so it should not take them so long.
