invoke hcp


what am I suppose to do when I get this order from the MD in LTC ? Please somebody answer my question.

HCP=Health Care Proxy
I would imagine that this order means the resident/patient is deemed unable to make medical decisions for themselves, and that their Health Care Proxy should be contacted for further decision making.
If you aren't sure, you could always ask the MD what they mean..

You will need a valid Health Care Proxy...make sure the person who is named is still 1.willing, 2.alive, 3.not in jail! You need an MD order AND a progress note explaining why the doc invoked the HCP. At one place I worked, we had a form the doc filled out which included a brief explanation of why it was necessary to invoke...such as severe dementia or coma...and if the invocation was going to be permanent or temporary.
At the facility I'm at now, we get an order and a progress note then make a copy of the progress note with the reason for the invocation highlighted and we keep it in the advanced directive section of the chart.

I thought you needed 2 docs to sign off on a HCP?
I could be wrong. I would also ask the SW

In Massachusetts it only takes one doc OR one NP.

All this confusion and bruha about health care proxies and who can sign the resident in and who can be told information. Makes me long for the old days...if you were married to someone, and most often you'd been married to them for 50 years or more, they could sign you in and consent to treatment. "Oh, so sorry Mr Jones but just because you've loved and been married to your wife for 64 years doesn't mean you can have any legal standing in the admission process."...I had a nurse who wanted to invoke EVERY resident's health care proxy to make it easier. Yikes! I had to explain it to her....just because you had the foresight to have a proxy doesn't mean you can no longer speak for yourself. I have one hanging on my refrigerator door (along with my own care plan just in case) but I'd kick anyone who tried to invoke it if I could still speak for myself!
