You will need a valid Health Care Proxy...make sure the person who is named is still 1.willing, 2.alive, 3.not in jail! You need an MD order AND a progress note explaining why the doc invoked the HCP. At one place I worked, we had a form the doc filled out which included a brief explanation of why it was necessary to invoke...such as severe dementia or coma...and if the invocation was going to be permanent or temporary.
At the facility I'm at now, we get an order and a progress note then make a copy of the progress note with the reason for the invocation highlighted and we keep it in the advanced directive section of the chart.
