It has been a few years since I worked with high fall risk and dementia patients and don't have any sites to offer. I will offer some guidelines though that I think may help.
Identify a fall risk
Tag that fall risk patient in some manner so everyone knows
Identify why they are a fall risk, get the physician involved in this early.
Correct the problem if possible, ie, medications causing delerium, sleep deprivation, out of normal environment, infection, head injury, dementia issues and make a safe environment.
Interventions can be 1:1, low bed, floor mats, restraints, medication, reorientation, family intervention, comfort items (back rub, familar music, personal items close by) , psych consult, physician interventions and nursing measures within your facilities guidelines.
Hope that helps
