Behavior management

Have any of you heard of any good behavior management programs and how are they set up? I'ts time to re-vamp ours and I'm just looking for sugestions or examples. The biggest problem we have is nurse documentation and helpfull/realistic interventions. Also havong trouble tracking/documenting psychotropic meds.
