How do you deal with making a med error? Even if it was an honest mistake?

Most med errors aren't on purposes.

You report it to your charge and write yourself up. Monitor the resident for problems r/t the wrong drug.

The worst thing you can do is pretend it didn't happen. I agree with PP. Write yourself up a med error sheet and report the error. Depending on exactly what the error was, will depend on what comes next. Many med errors cause no harm.. You can get vitals and there won't be a change in condition.. Of course others.. you get the picture.I've made 3 med errors, that I know of, in the last 4 years. I knew myself that I did them. Thankfully, there were no ill effects from them.

Use this as a learning experience! Why did you make the error.....similar medication......distraction..........not following drug checks......complacency? Work out why and learn from it,it happens to us all at some point,we are only human after all!

We are supposed to get VS, notify MD and see what is ordered - labs, whatever, make out a Med Error Report, and monitor the patient.

Admit it at once.

Take care of the patient.

Take care of yourself.

Take care of the paperwork.

Hopefully, all errors/mistakes are "honest". I cannot imagine making a deliberate one. Resolve to learn from the mistake and move forward - don't get stuck. Improve your professional practice from the lesson.

Med errors happen, report them. Proper reporting makes for new policy and procedure to prevent them from happening again.
