I especially enjoyed dsczephyr (Denise's) response to Sample Question #2 and how she deduces the answer that she does.
It can all be done very logically.
When assessing a patient, which symptoms would support the nursing diagnosis of fluid volume deficit?
A. Urine specific gravity of 1.015 and thready radial pulse
B. Straw-colored urine and decreased skin turgor
C. Thready radial pulse and straw-colored urine
D. Decreased skin turgor and a urine specific gravity of 1.035
