Nursing Students General Students

I am nursing student still working as a cna in a hospital setting. I would like to get some good examples of nursing report sheets before my clinicals start in a couple of weeks. I cannot seem to find good examples of what is being used. can anyone help?

I am attaching a document file with my report sheet in it. If for some reason you are unable to open the file or access it, you will have to send me a Private Message with your e-mail address in it so I can send this to you. I tried to send you an e-mail through this forum, but you have disabled that function.

I am also attaching a new form I have been working on for a student to use during her clinical sessions. Any feedback would be appreciated as the form is not completed yet.

Im an RN in IMCU/ICU and I have sheet i use. I pretty much use most of it if not more. I cant attach it, im not sure how so I can email it to whoever wants it. Jen

Hi Daytonite,

I really like the doc you are working on. Good Flow. I would add in code status, and precautions/safety issues. On the meds part, a nursing considerations column would be nice.

I've said it before, and I'll say it again. You would make an AWESOME nurse educator. Many of us here on this forum have benefited from your experience & wisdom. A BIG thank you for all you do for us!:flowersfo

A couple of things we are required to record on our school's form:

  • Pt room #
  • Lab values (they gave us the list of which labs they wanted us to know), which includes a column for the pt's #s and then one to put the normal values in for an instant visual
  • A Pain/Discomfort section to record info
  • I & O information; with diet, % eaten, N & V, PO or TPN, PPN, info

Also, we do VS at least twice during a shift, so a place big enough to record those and possibly a 3rd set if they're q4hrs (sorry, I had to close it, so don't know if that was there...)

Thank you Daytonite for the sheets! This student one is in the order that we are supposed to give report so I have saved a copy and will be using it frequently.

I have a sheet that is very basic for assessments. I do meds on something else.

But I will try and attach a copy of my assessment sheet. It's an excel spreadsheet. If it doesn't attach I'm more then willing to email it. There were changes that I was going to make on it and I believe they were done so it should be complete though always open to changes...

I am working on my meds one and will have that done in about a week or 2. I plan to have it done for next semester since I only have 2 more clinical days this one.

My meds and assessment sheet will likely be printed double sided so that it's 1 sheet of paper. I also print them on yellow paper because it's easy on the eyes. The assessment one is designed to be folded in half so that in the morning you can see what each block is for and same with afternoon...no flipping. The only reason I may not do it double sided is because of the design of the assessment sheet to be folded in half. If I do them on different papers then the assessments will be yellow and the meds will end up being a different color just for ease of recognizing them.

Okay never mind it won't upload because it's excel. If anyone wants it I'm more then willing to email it. Just let me know (and if you post on here that you want it just let me know it's mind that you want since there are a few posted...you don't need a bunch if you only want one).

I should also add that I have yet to have a diabetic pt which is one reason there's nothing on my assessment form for that. I'm not sure how I never had one but somehow I keep missing them. I will likely put it on my meds sheet though.
