Really bad test questions!!

Don't you guys just hate it when there is a terrible test question! Here is a question, that I think sucks!!!

I can't remember the exact wording--

- You know that your patient has fully understood how to perform his creatine clearance test when he makes the following statment:

A: I will cath myself to get my specimen

B: I will completley empty my bladder into the container

C: It is okay if I accidentlly flush some urine down the toliet

D: I will collect all my urine for 24 hours

So--I picked B. However, my teacher says the answer is D. But, in a creatine clearance test you are supposed to throw away your very first morning urine (or at least that is what we were taught) My teacher admits that the question is bad--but wont give me the 2 points. I just don't understand how D can be correct, when you are not supposed to collect "all" of the urine.

Any insight?

That first morning you do urinate into the toilet and that specimen is not counted but every specimen after that is counted and collected until 24 hours later. So B would not be correct because you are not completely emptying your bladder into the container because the first time you urinate you do not count that. D is correct because after that first morning urination, you do collect all your urine in the container thereafter.

the stem of the question states
your patient has fully understood
how to perform
his creatine clearance test.
the test is a 24 hour urine collection. that is how it is performed. answer b is merely one part of the performance of the test and not indicative of the entire test. answer d would indicate that the patient understands that they will need to collect their urine for an entire 24 hours.

As with many students, you are reading more into the question than is asked. When taking your tests, please remember to read ONLY the information provided--don't ASSUME more than is given. This question is asking you to evaluate the patient teaching for a creatinine clearance test. The important thing for the patient to remember is to catch all the urine for 24 hours.

Sorry, seems like a simple question to me. I would have picked D.

However we never have had a quesiton that simple and straight forward, I would have probably second guessed myself, and thought it couldn't be that simple

i also would have picked "D"- it is a 24 hour urine, so you would do the 24 hr coleelection standards- and yes you would collect all of the urine in a 24 hr period- you discard the first urine and record the time from then until 24 hrs

I immediately chose D. I don't understand why you picked B. The pt indicated that they understood that they are collecting 24 hours worth of urine. It's very straight forward, actually.

I pick A, I want to see the Pt. try and cath themselves. Ask them to youtube it :p

Well...they way the procedure was taught to us was...if you start at 9 am--you void, discard that urine and keep the rest until 9 am the next morning. So, I guess I am just being literal..when it says collect ALL the urine. If you throw the first one out, you are not collecting ALL the urine.

I still think it is a bad question. And, my teacher did too---cause she gave us the 2 points!! She accepted both B and D. :)

You discard the first void, then all urine produced in the next 24 hours by the person is collected. The timing begins after emptying the bladder to ensure accuracy.

Bravo for you!! I think it was a rotten question too.... we had our professor end up giving us 4 points in total on the 1st exam due to questions like these... it helped a few people to make it to the passing mark...
