Hi I am having trouble with my nursing care plan. It went well last semester but my current clinical instructor isn't quite fond of me, so wanted to bounce it off anyone please.

Admitting dx: altered mental status, hyperglycemia.

Nursing diagnosis: Altered mental status related to management of medicine as evidence by high blood glucose level.

And I do pathophysiology on DM
