need help with Risk for suicide diagnosis!

I wrote my risk for suicide diagnosis but I dont know if it is correct/makes sense! Please Help!

Risk for suicide r/t past history of suicidal attempt, current suicidal ideation and thought out plan, expresses feelings of sadness and hopelessness secondary to substance abuse, chronic pain, and MDD AEB verbally expressing suicidal ideation comments such as "I don't want to live anymore"

Do i have too many r/t factors? should i cut this down or is it ok?

It sounds like some of your R/T are your A/M/B. I'm still new at this, but what are the stressors? I would think those are your R/T as well.
