missing exam mark - what next?

I guess I'm hoping for viewpoints about how to think of my future and my RN re-entry program. My course facilitator lost my mid-term exam and has not recorded any mark for me for this mid-term (that was back in Feb and I'm still waiting). I did the assignment and wrote the final exam, to be told by staff that it appeared that I had not written the mid-term exam. My facilitator did phone me to explain that she had lost my exam. But it has been a month since the course finished and there is nothing to indicate that I have the course credit. I paid my tuition fees, I did the work with excellent marks, and I completed the course. At this point I am relying on her memory to do something about this. Meanwhile I have started the next courses in the program. Problem is, I have become so disillusioned about the thought of this not being resolved, and of these kinds of things happening in future - I'm not so sure that I even want to continue in this program at this educational facility. They had changed the program just before I started, the exams are a bit of a mess. I am a person who works hard, I have done many courses, with a couple of university degrees - is there a way through this?

Can you go up the chain of command? Try to speak to director fo the program.
