Heparin injections

  1. Yesterday I administered my first 2 Heparin SQ injections. I am used to giving insulin injections where the needle goes in like butter. I used the same technique I use for insulin injections for my Heparin injection I had a difficult time getting the needle through the skin with the Heparin. Does anyone else meet a lot of resistance to getting the needle through the skin when administering Heparin? Is there a technique that will make it easier? As of now I am scared to give another Heaprin injection because I don't want to cause excessive pain to my patients.

    I did a study once that showed when you draw up heparin with the small needle, you blunt it considerably. So I always drew it up with a regular large needle, then put the small giving needle on. This also prevents extra bruising at the entry site from heparin around the needle.

    If you're talking about prepackaged sc heparin syringes, I can't remember how easy/difficult they were to insert.

    all the hep I have ever given comes in a pre filled syringe. Pinch up the skin good and pop it in there, use a good POP to get it in. I once witnessed my primary not give it a good poke and it actually bounced off the pts skin (she was a tough ol hide) and sail across the room.
    Go down at a 45 angle and hold it like you are holding a dart.

    This is how I always gave it. One particular time, my instructor was observing my med pass. I was thrilled when, after the shot was given, the patient asked it I was already done because he hadn't felt a thing! He then proceeded to tell us that he always dreaded the Lovenox shot because it hurt so badly. That was a great clinical day!
