I am trying to do my care plan that is due tomorrow and I am stuck on the nursing diagnosis. If anyone can help me I would appreciate it. My patient came in the hospital for a possible bowel obstruction. His other medical dx is asthma, dyslipidemia, hypertension, hypothyroidism. I already did a care plan on fluid volume deficit but I cant figure out another nursing diagnosis to come up with. I am hoping there is some one on this site that can help me so I can get my care plan done tonight. Thanks.

Why are you doing fluid volume deficit when there is nothing in the medical history about this dx? Nsg dx for pssible bowell obstruction could be keep NPO,Asthma could be manage, minimize complications of,HTN could be B/P will be controlled or pt will verbalize understanding of need to monitor diet for hidden sodium, or and will verbalize understqnding of need to take medications on regular schedule.Good luck !

Patients with bowel obstructions are at risk for ileus. With bowel obstructions patients third space fluids in the intestines. They get distended. This fluid can never be absorbed back into the body and is considered lost fluid and can be considerable loss of fluid and electrolytes. This Deficient Fluid Volume is a perfectly legitimate diagnosis to use for this patient. All the swelling and distection can cause pain. Acute Pain.

Besides the ones given, you could add Risk for Imbalanced Nutrition: Less than Body Requirements, either from the obstruction or from the fact that he's probably NPO while they figure out what's going on with him.
