It seems to me that when nurses work with them in private practice, there is a conflict, because the MA worked under the physican, yet, the nurse has her own license, but may be responsible for what they do.
I'm guessing I just don't understand the problem you're having; I'm certainly not aware of any uproar or even any disharmony.

Also, you, as an RN, might have your own license, but if you boil it down to it's essence, it is essentially a license to follow physician orders. We don't have the market cornered on ability to follow physician orders and I doubt we ever will.
Bottom line, in this profession, is that you are never
responsiblefor actions taken by someone else. You might be held
accountable for their actions if your role is to oversee said actions. You are neither responsible nor accountable for the actions of another person that you do not supervise.
