Not an order...but lack of.

Admit for "pain control" for a hip fracture...with elevated INR. Repeat labs in am for possible surger.

Meds ordered....po tylenol 650. When I called to get something more for this "pain control"....I got a very crabby dr telling me the surgeon will follow in the am. (of course I had to insist on SOMETHING for pain before am)

I like the ones where the doctor thinks if he orders it, nurses have to do it. For instance, newly placed dobb hoff tube...md wrote order "Start tube feeding now, xray to follow". He specifically wrote that because I told him the feedings could only start after xrays verified and he wanted it started NOW. Yea, ok...xray showed dht not in place and had to be redone.

He probably wanted experience in treating a pneumonia patient? I once had a doc order to start the chemo on a newly inserted central line without an xray for placement. Of course, we refused. Good thing, too; the line had made a wrong turn and was in the patient's head...

Ambulate patient 4x/day. Obtain patient's shoes from home.

For a wheelchair bound double BKA (and no, he didn't use protheses).

This is on a post-op standing order sheet for one of our surgeon groups: "NPO until extubated and alert."

Found this telephone order while doing chart check on a pt transfered out to our m/s unit. It was written by an ICU RN:
Nystatin powder to intriginal folds qid.

I think them there intriginal folds need-a-powderin' ....

Hmmm. I got an order for NPO except ice chips.. for a pt on a vent. Also, called in labs and condition report on a post op carotid--- sliding scale potassium. Huh? ~Ivanna
