Hello,

I am on my quality and safety committee in our PICU. Our current policy is to change out tpn to the hub every 24 hours. This means we are opening up the line every 24 hours. A nurse has told me that our NICU has been changing their tpn every 96 hours. They still change LIPIDS Q24 hours. Their CLABSI rate has decreased but they implemented other interventions.

According to CDC, "In patients not receiving blood, blood products, or Fat emulsions, replace administration sets that are continuously used, including secondary sets and add-on devices, no more frequently that as 96-hour intervals."

I have been researching this ever since someone brought it up and have not found much information on the topic. Just wondering what everyone does at their hospital and if you have changed your practice to changing ONLY TPN to Q96hours.

We change out tubing every 24 hrs. It's interesting to note, however, that the central line the TPN flows through may not get changed for weeks. Kind of makes how often you change the tubing a moot point.
