state to state.different hosptial cost?

I was curious...I know the reason for difference in pay from state to state is because of cost of living..but shouldn't there be some kind of standard pay rate for an rn...I'm just amazed at how someone in one state could be making $17 an hour while someone in California might make $45. Its a really big difference. Does hospital procedures not cost about the same for different hosptial, is it based on the hospital's productivity? The pay is obviously not always based on the need for nurses..

    I'm just confused thats all..you can be a pharmacist and know that no matter which state you work in you will make $50 an hour...you can work at mcdonalds and know you will make $6/7 an hour...
