I'm working on a interprofessional clinical learning unit and we are trying to develop a new kardex for the nurses to use. Currently there is a 1 page kardex however we are looking to develop something similar which would have a multidisciplinary or interprofessional component. This tool would still largely be for the nurses but may have relevant areas for other disciplines...I'm not sure if this makes sense but basically I'd love to recieve any kardexs out there.. and hopefully we can help adapt it to what we are looking for. The ward is a acute care of the elderly/stroke ward. Let me know if you have any kardex I could take a look at or if you have any questions.

Before the computer age, in the time of paper, when a non-nurse wanted info... they just asked us. Talking. It works. =)

I know some people swear by Kardex, and that JC likes to see them being used, but I never trusted those things to have up to date info. We do not have a UC at night and are so busy that the Kardex is the LAST place to look for best info. Sorry, not what you asked for. Have you asked reps from other disciplines what they would like to see on this form? And if they would actually use it? And could they find it when so many other people are shuffling for the same info in the same place at the same time?

We use a Kardex that is pretty good... depending on what unit you're on, as some of them don't use them at all. I like them because they have the current diet, lab orders (not results), and other tests that have been ordered. They used to be better because the most recent VS were also printed.
