post partum care plan

Can someone please help me with a postpartal care plan. I am using Knowledge Deficit related to limited experience and skill in providing infant care and self-care after childbirth. I am having trouble with assessment interventions. I am not sure what I can use. This is her first baby and she has not had any other experience with children. Does anyone have any suggestions?
