PLZ HELP!? I need help with an assignment

PLZ HELP!? I need help with an assignment


Please help with an assignment i have! I don't have anyone that i can talk to so im reaching out and hopefully get some help with this assignment. The following is what i have to do for class:

Project: Medication error
Instructions: Interview an RN/LVN who dispenses and passes medications in his/her work. (acute or long term care)
Questions:
1.Has there been an instance wherein medication errors (wrong drug, dosage, patient, time) were done by such Nurse or another nurse in their place of work?
2.What was done afterwards? Was the patient notified? What happened to the patient?
3.What are the institutional policies set forth by their place of employment to correct or prevent such errors from happening? What are nurses doing themselves to prevent such occurrence?
4.What are the state and federal regulation laws that a nurse is liable for if ever medication errors occurs?
If an experianced and working nurse who does med's could PLEASE help me, i would REALLY appreciate it. There doesn't have to be any names, of course (respecting HIPPA rules), of any sort. Just broad terms, like "patient" "facility" etc....It would be a great help if anyone could help me! Thank you!

2.What was done afterwards? well, the dose is then called over to the pharmacy to time is changed on the MAR, or simply the dose is just missed and it is given when it is due next. there is nothing else you can do. Was the patient notified? no, pediatric patients, or simply the parents do not notice. What happened to the patient? absolutely nothing. the medications then continue as scheduled.
3.What are the institutional policies set forth by their place of employment to correct or prevent such errors from happening? well, nurses who have committed the error are notified, and have to sign a paper stating the circumstances of the error, and it is sent to risk management. What are nurses doing themselves to prevent such occurrence?well, there are many in-services and teaching modules that are updated every few months on preventing errors. the clinical nurse specialist will teach nurses or reinstate about time management, not to miss meds, always double check the meds and use the safety measures in the hospital; like the computer medication administration program on the computer, where the nurse scans her id, then the patient, then the medication, then the medication is administered.
4.What are the state and federal regulation laws that a nurse is liable for if ever medication errors occurs? well, that you have to look up in your state board of nursing. because all nurses must act according to the hospital policy, therefore all the consequences must be done depending on the med error. such as if the med error caused anyone's death. I have heard stories where the wrong dose of a medication was given to a patient, and the patient passed away; the nurse lost her license in that case.

You really do need to do some research on your own, too. Especially on what the laws are about med errors.

Some interesting reading for you might be to go to the website of NSO and read about their cases that they represent.

Hi, I do have the laws on med errors, just looked them up and read them. Just needed a RN/LVN to help me with things that i needed answered personally. And how do i get to NSO? I would love to read the cases, i googled it and some other things but not sure what you meant
