Please help me, I'm in trouble.

Hello. I have been a nurse for about eight years. I have been employeed in a skilled rehab unit for about five years. This past weekend I made a serious med error. I gave a resident another residents medication. I administered oxycontin 20 Mg instead of the ordered oxycodone 10 '=: This was realized at change of shift. As soon as I realized the error I assessed the ot and contacted the doctor as well as told the nurse assuming care of this pt. I told my supervisor that I had made a med error and she said make sure you call the MD. I did not go into details as it wa already two hours past the end of my shift, and you do get written up for being late out. Not to mention that I had twice as many patients as I should have due to staffing problems. Later that same night the supervisor calls me for clarification. I did not fill out the proper paper work. More importantly I wrote in the drug manifest "Wasted" To adjust the count from the pt that I took the meds from. After I wrote that I thought well its not actually wasted so I documented beside my signature "Given to wrong patient" In a very small space. Well long story short a few days later my DON questioned me statng that I falsified the document, I was being terminated and reported to theboard. I am in complete shock.. she said it looked like a cover up. But remember I told the other nurse and called the doctor and no harm came to the patient...thank god. Now im out of a job, will face the board and I don't know how to get a job after being fired. Please please help me.

Pls be careful what you share here, Shelley especially if you feel your former employee is reading your posts. Just trying to look out for you. If you are going to proceed with any action against anyone do it off the board and don't report it here. And you can go back to court against your hubby to get child support. That order can be re-opened if things changed. They can re-issue a new order with child support and put in writing who can claim the child. You can file yourself through the courts.

the med error did NOT kill the patient. It was a PO med (not IV Dilt or anything). Even if you forgot to chart your follow up with the doctor, you informed the next nurse, the doc, etc. It was not an INTENTIONAL or LIFE THREATENING error. I would calm down. We all make mistakes and learn from them. The error sucks, but remember, it was not a life threatening med error.

Thank you. The actual error is that I cave oxy er (Oxycontin) rather than plain oxycodone. So in all actuality she got less narcotic but over 12 Hours versus 4 Hours. No harm came to this patient whatsoever. Yes it was p.o.. her justificationis over the narc log that I noted wasted on the other pts account, as in not given to her, I also noted in the margin it was given to other pt. So falsification is bogus. Did u know that falsification of a narcotic log is a fellony which involves the DEA? I recently learned that and since I know the DON S goal was and is to burn me I suspect that is why she chose to use the word falsification. I also offered a drug test to prove there was not an issue of diversion and she declined and said that had not crossed her mind that she knew the alert oriented pt did receive the med.

Positive thoughts to me at this point. I have a job interviewe lined up and its actually my dream speciality. Don't want to give details here so that her evilness doesn't carry over to the other possible position. Wish me luck and any pointers on how to disclose this on my interview would be helpful

Also I am very fortunate to have my most recent employee evaluation which is awesome, as well as several letters of recomendation from some impressive people. Again I wont say here what their positions are to protect myself and them, but let's just say when this goes to the unemployment or hr review the DON will be shocked.

you didn't falsify. Perhaps you charted not in the way they would have liked on the MAR, but you did chart it was given to the wrong patient, which is not falsifying. You can't help that the DON is stupid. The DEA would laugh at her, like the BON will, if she is stupid enough to make a claim about you.

You know, this is not such a good idea if there is any kind of legal procedure in the wings. I went thru nursing school while I did a child custody battle, + 18 units/qtr at a UC. It wasn't any fun. Just be careful, my friend. You would be surprised what ends up on paper that was just "pillow talk".

BTW, I'm not on facebook, not a listed phone number and take every precaution to keep my name private. Guys - be very careful, everyone.....The social network was not a great fad.....it's dangerous if there is someone out there who has something against you. I had my house broken into, and I stopped sleeping at night...the police said that the crime scene "looked personal". Like someone had been following me or it was someone with a grudge. It didn't set in until later....and then I was REALLY scared when any crisis came around. We can't help anyone else unless we, ourselves are protected.

*****UPDATE*******

Well I had the meeting today with the administrator that I respect. I got my job back, I was wrongfully terminated! Corporate HR was told that there was a ton of disciplinary action in my file which is false but is why I was actually terminated. I did receive a suspension that has been served. What do you think? I'm sure that the DON will really have it out for me now as she is in trouble bc she in all actually falsified documents, my file. At least this way I can find another job and l can leave on good terms, MY TERMS, and I will not have a termination or board action investigation against me. Thank you all for your support! I would appreciate any continued guidance on this matter.
