Looking for Fall Risk Assessment Tool

Hello,

One of my nurse educations would like to find out which Fall Risk Management Tool you use that includes: age, history of falls, mental status, physical mobility, communication/sensory impairment, elimination, vitals, medications and diagnosis.

Any suggestions would be very appreciated. Home-grown or purchased.

The MAHC 10 addresses most of these. I do home health and we have the option of this, Tinetti, and TUG

I think Briggs catalog has a form ( or more) that NHs use. I know we've used pre-printed ones at the various facilities that I've worked.

Briggs has all kinds of forms.

WE used the Morse Fall Scale, I don't remember if it includes vital signs, we would monitor them on a separate form.
