IV administration questions

I am working on revising our current IV policies for PICC, central lines, and peripheral lines.
I have some questions that are not addressed in the standards I reviewed and wondered what you do at your facility.

Do you dilute meds before giving IVP through a running IV? how about an adapted IV?
Can any nurse access a port at your facility?
Do you use a preprinted order sheet for IV's--define type, if you can use if for various things? flushes and volumes and frequency? This is more pertinent to central lines and picc's. If not how do you get an order--do you ask on admission or is it a protocol? would you be willing to share?

Who puts PICC lines in at your institution?
Do you use prefilled flushes?
I think that is it for now--any input would be greatly appreciated.
