Incident report for injury with unknown cause?

Hey all,

I worked 11-7 last noc, and woke up this afternoon with pain in my upper left back. Not sure if it's a pulled muscle or pinched nerve, but it sure hurts when I move my arm in certain ways, or lift laundry detergent (even with the right arm) Doing the usual heat, ibuprofen and muscle rub, will follow up with my primary if I need to. NOT looking for medical advice here ;)

The question is, if I'm relatively sure this was caused by something I did at work, but I can't recall a specific incident, am I supposed to report this to my employer? I only ask because they've made such a huge deal about reporting ANY work related injury, no matter how minor, yet I don't have a clue what I'd report.

:typing Think back - did you lift anyone out of bed? Change someone by yourself - Try to remember all you did the night before.....I once hurt my back helping a L.O.L. who couldn't have been more than 110lbs in and out of bed 100x's because she had 'urgency' and wanted to use the bedside commode....well, I hurt myself because she needed alot of help even though she was pretty small..... After that I won't ever do that again - I need to share that task with others and not do it all by myself.....:nurse:

Yep, figured it out. I did a pivot transfer stretcher to chair with the help of another nurse. I was supporting the pt with my left arm, and she was a bit unsteady. I remember her saying "I'm sorry I'm leaning on you so much"... shoulda been my first clue.

my advice would be to initiate a claim with ur manager ASAP. the longer u qait the less likeky they ate ro accept full fuduciary responsibiliy. I've watches s staff in 2 weeks put in claims. were fake
