Clinical Practice Guideline

Hi all,

I was wondering if there is anyone out there that would have a clinical practice guideline or protocol for addressing medication adminstration errors related to nursing.

We fill out an anonymous reporting sheet that indicates what medication was given, when, and what the errors surrounding the administration was - for instance did the medication get cancelled but the order hadn't been scanned to pharmacy so wasn't in the computer, or was the time changed, or was there a few sound a like meds given and you picked the wrong one, etc.

Of course the doctor must be notified for further orders.

Hi all,
into my 2nd week of placement in acute care.
not really comfortable in looking after demenitia patients.
can any one give me some advice on how to help them as a nurse.

Have you already looked at www.guideline.gov? That's the National Guidelines Clearinghouse that has a database of more guidelines than you can imagine! I just did a search for "medication errors" and came up with a bunch: http://www.guideline.gov/search/sear...+errors&num=20. Might be what you're looking for!
