Can anyone help with NCLEX Candidate number?

Hi I have already posted this on the graduate forum but got no replies.

Like all things you put in a safe place until needed and then cannot find :D

I am hoping to resit the NCLEX exam after a 2 year break but with another nursing board, on the form I have to inform them re previous tests but unfortunately cannot find 1 of the 2 profiles. Will the candidate number be the same and if not does anyone have an idea where I might get this information.

In the meantime I will continue to tear the house up looking for the form. :rolleyes:
