Any Bristish RGN's or Students that can help?

Hi,

I was just wondering if anyone could help, I have to hand in my CAP (Continual Assesment of Practise) document, in the new year. I have completed all the required learning out comes for this semester, and had them signed off for by my mentor, as well as some others. My problem is that I don't have a seperate piece of evidence for each learing outcome, Do I need it?

I have started to 'collate' information from other sources such as a pain chart. However I just wondered if I needed to do all this extra work, as they have been signed for. As my mentor saw me filling in care plans, I had not included an example, and I cn not copy the origionals as it would be a breach in patient confidentiality, I can not get in touch with my Uni, till the day my CAP document needs handing in, any advice would be greatly appreciated.
