Lightbulb Transfer of case management responsibility

Our facility is currently moving discharge planning into the hands of the staff nurses. This was previously done by our case manager. I selected this topic for my senior thesis since I perceive a problem brewing. Please let me know what is going on out there currently and if other facilities have embarked on this route. I could use suggestions on problems encountered, implementation and education for the staff. Thanks for your help!
