Question medication errors

Hi guys I need your help! Im a student at CSN and i need to interview a nurse manager/supervisor about medication errors. If you are one or knows someone who is, please share your views on the following questions: No personal info is needed, initials are fine for privacy.

1) What is the frequency of medication erros in their(your) area of responsibilty?
2)What are the nursing responsibilities when an error occurs?
3)Has the nurse(you) had to develop educational plans or give letters of counseling because of medication errors?
4)What hospital committee makes decisions concerning med errors?
5)What advice does the nurse have for new graduate nurse?
6)How are medication errors traced by nursing to find causes and improve client care?
7)How do you encourage nurse staff to decrease or avoid med errors?
8)How do you respond to a nurse who made a med error?
9) What are the common med errors you see in your area of responsibility?

I need as many replies as i can! thank you so much!!!For those who want to type their answers on MSword please email the file. Again thanks and i hope someone can help me on this.

ooh mini i remember those questions.... is it for professional topics? i graduated from that program in may when it was ccsn. i am now a RN . good luck in school. keep me updated on how your doing...

hi! hehe no its for my pharmacology class..=) sure i'll let u know, so far im overwhelmed coz its my first semester but hopefully ill get used to all the work load soon.. thank u so much for replying! if u can share any experiences pls dont hesitate!=) Good luck on your career!=)

oh phramacology .. ooh yes that right... i did do that for phramacology... i knew i did it.. but couldn't remember the class... ooh yes keep me updated on how you are doing. i am so happy you are in the program. it is a great program and gear you toward boards.. they have a high pass rate and you will do well. good luck and hang in there. you will be writing RN after your name sooner than you think.
