job description for "disease manager"

I am interested in any thoughts on a job description for a position entitled "Disease Manager". This nurse would work out of a community hospital, establishing disease management programs for patients with chronic diseases. The nurse will work with private physicians' offices, public health department, and walk in medical clinics, etc. to promote continuity of care across the continuum for patients in this community.

A Disease Management nurse would work with a population of patients with certain chronic diseases to use well documented strategies in educating the patient, (and physician?) in avoiding complications, and staying as well as possible. Use your "search" function and enter "disease management" for lots of info.
