ER Case Mgmt help!

I am in a new position as CM for the ER. This is a new program for the hospital with the main focus on: identifying admissions, appropriate IP or Obs, initiating Core Measures, and at a later time on through-put times. I would love to talk to someone who is already doing ER CM and get their perspective, plan of attack when they arrive into the ER to begin their day. My background is ED nursing, Quality Mgmt, Pt Safety & Risk.
Please help as I feel that I am spinning my wheels much of the time.
