Hi,
Does anyone else have Case Management Associates at there Hosp?? We have them, they arrange the SNF placements, transportation, arranging home health IV ABX's, Wound care, Home pt and DME delivery arrangement and set up, Rehab referrals in state and out of state, they arrange medical air transport state to state, down to cabs. Medication authorization and pre-auth for other services needed. We talk to the pts and family to make sure they are ok with the discharge plan and are available to help when needed, and also do the UR's and other clinical issues. We're lucky!!
