DD resident in LTC

I have a fifty-something year old resident at my nursing home who wants to live out in the community.

His Regional Center Case Manager told him he can move out if he has a Medicaid waiver, but he can't get a Medicaid waiver until he is assessed, and he can't get assessed until he is living out in the community, so he is basically stuck.

Is there anybody out there who has helped someone in a similar situation? I can't bring him home; too many stairs.
