Re: Pretty basic question from a student

There are two reasons for heart murmurs: stenosis (forward flowing blood trying to get through stenotic valves) (occurs when the valve is open) and insufficiency (blood flowing backward though an incompetent valve) (occurs when the valve is closed).
The valves that are open during systole are the aortic and pulmonic and the valves closed during systole are the mitral and tricuspid. Therefore, aortic stenosis is a systolic murmur and mitral insufficiency is also a systolic murmur.
During diastole, the aortic and pulmonic valves are closed and the mitral and tricuspid are open. Therefore, pulmonic insufficiency and mitral stenosis are diastolic murmurs.
I hope this helps. It's a confusing concept.
