Cool Peri-operative Care Coordinator

This is a new position I'm trying to get implimented at my place of employeement. This person would be responsible for all Pt. teaching follow through hosp. stay and follow up after D/C for all post open hear Pt. Does your facility have such a person and what all does this person do. Thank you

I work in an 8 bed CVICU unit. We have no such person here, and have not heard of other hospitals in the area the have one either. Sounds like a great idea!! Please keep us posted.
