I read with interest the discussion about med errors. A recent question at our institution relates to those medications that are routinely double-checked by another RN. Some nurses have experience at other hospitals where the practice is different. What is the standard of care for this double-checking? What is the standard practice at other hospitals? Some medications that have been suggested to be double checked are:
insulin, heparin, pediatric medications, TPN, chemotherapy
