#1 safety peeve.

My #1 pt safety peeve is illegible (what I think of as culpably negligent) handwriting, and illegible signatures without printed names under them.

I hate trying to make out what the orders are when doing 24 hour chart checks. Sometimes it takes 3 of us to figure out what the scribbles mean. What a waste of time. Is it THAT hard to write so we can read it? It doesn't have to be all neat and pretty, just so we can read it....

My pet peeve is that it seems like whenever we have a patient transfer from another unit, we find what look to be errors in orders.

I've given up and I go over the entire chart whenever we get a transfer, which nearly beats a direct admit for complexity.

Safety peeves in long term care:

Overfilled Sharps containers!!!!!
Door to med room not locked
IV lines not labeled
Sticky G-Tube feeding on Pump stand and floor.
