trach suctioning

have you ever heard of suctioning a trach on a child and their lung collapsing? i have never heard of it , but it happened to a friend of mine's son... i am a Rn and have suctioned many trach's and have never had a lung collapse...

is this possible? How old of a child did this happen too? is there something WE can do as nurses to be prevent this?
