Need Information - documentation/signature

We use charting in our ICU, where we have a flow sheet where we document in the corner, our initials, printed name and signature.
Our narrative is done on a separate paper....we have different view points on the following:

Does each entry have to have a full signature, with credentials, or can we sign with initials and credentials?

We always end each entry with initial. But at the end, the last entry has to end with our full signature and credentials so that its known whose initials it is.

We have always been told that each narative entry must have our signature et title.

My first entry on the page, I sign my name and title. After that, just initials for additional entries.

Each page of our flowsheet has a place at the bottom for all personnel documenting on the patient to sign their full name, credentials, and their initials next to it. Throughout the rest of the page, we use our initials.
