I would like to acquire your guidance as I have to incorporate physical examination / systemic examination in Psychiatric Initial Assessment Form. The existing form contains a detailed psychiatric examination as per JCIA documentation standards however, deficiency is found in this mentioned standard. Physicians perform systemic exam of the patients and they document in progress sheets but sometimes they forget / or reluctant to write. Now, in order to comply with the standard I have to include it into physician initial assessment form of Psychiatry. I would like to have your suggestions that what components may be slot in as part of it or any websites that contains information regarding this? Thanks in anticipation.
