Letter of reference from EMT job

I'm probably going to be leaving my EMT job soon. My manager let me know he'd sign any letter of reference I gave him and would give me a good review over the phowone to anyone who called. It's up to me to write the letter.

I want a position as an ICU nurse in a new-grad program. How would you word a letter for this guy to sign? He's entirely satisfied with my work, it's just that one-day-a-week employees are about to be cut.

I will have letters of reference from three of my clinical instructors plus a letter from my EMT supervisor.

Thanks for any suggestions.
